Pre-K Registration for the 2012-13 school year opens on Monday, March 5.
Children must be 4 by September 1, 2012, in order to be eligible for application.
Parents should go to the elementary school within their assigned district to apply.
The following items are necessary for a completed application:
- Proof of Age Eligibility – copy of certified birth certificate or other acceptable documentation such as military ID, passport, adoption record, or affidavit for establishing residency and age.
- Certificate of Eye, Ear and Dental Exam (Form 3300) or appointment card
- Certificate of Immunization (Form 3231)
- Copy of Social Security Card
- Proof of Local Residency - copy of one of the following items with your name and correct address: utility bill, trash pickup bill, lease agreement, home owner’s insurance, etc.
Application to the Pre-K program does not guarantee acceptance. Acceptance will be based on a first-come, first-served basis.
